Firearm and Shotgun Licence Applications

All applications for a new shotgun, firearms or explosive licence, will be required to provide a GP report which verifies patient medical history against the relevant conditions. Applicants must download and have their GP complete the Medical Proforma before submitting an application, without this no licence will be granted.

This will also apply to all applications to renew a shotgun, firearms or explosive licence after the 1st February 2020.

Applicants can download, from the police website, the Standardised Medical Information Proforma associated with the licence application.

The York Medical Practice will complete the Medical Proforma in relation to an application for a shotgun/firearm licence or renewal of a licence, for which the fee is £80.00.
