New Patient Registration

The York Medical Practice welcomes new patients. In order to make the registration process easy we list below the documents that we need to see;

  • An official form of identification e.g. passport, driver's licence etc.
  • Proof of your address e.g. household utility bill (Gas, Water, etc.). Bank or credit card statements can be shown to ensure that you are within the practice boundary. If you do not live within the practice boundaries we are still able to register you - please see information below on Out of Area Patients. 

It is practice policy to request all new patients to complete a health form. This will give us an outline of your health prior to receiving your medical records from your previous doctor. Once your forms have been received we ask for 24 hours to process your details, after this we will be able to book an available appointment with the clinician of your choosing.

Out of Area Patients

The practice is now able to register patients from outside its allocated boundaries. If you are registered at our practice under our Out Of Area Contract (OOA) you are not eligible for a home visit and you may be limiting yourself from other urgent out of hours services provided by the practice.

Under our OOA contract The York Medical Practice will be asked to review patients that are signed up to this service and the frequency of its use. Depending on the patient and their medical needs the practice may recommend re-registering to a more local GP practice to the patients home address.

If you have moved, or think you have moved out of our practice boundaries and would like to stay please update reception of your new details and will will provide you with some more information. Please either visit us in Twickenham, call the practice or fill in the 'change of details form' on our website. Change of Details
