New Patient Registration
The York Medical Practice welcomes new patients. In order to make the registration process easy we list below the documents that we need to see;
- An official form of identification e.g. passport, driver's licence etc.
- Proof of your address e.g. household utility bill (Gas, Water, etc.). Bank or credit card statements can be shown to ensure that you are within the practice boundary. If you do not live within the practice boundaries we are still able to register you - please see information below on Out of Area Patients.
It is practice policy to request all new patients to complete a health form. This will give us an outline of your health prior to receiving your medical records from your previous doctor. Once your forms have been received we ask for 24 hours to process your details, after this we will be able to book an available appointment with the clinician of your choosing.
